Speaking as an epidemiology doctoral student currently completing a journal article on the subject, viral suppression == 0 chance of transmitting alone is not (necessarily) enough to guarantee elimination of ongoing transmission. There are two additional issues that must be addressed:

1. Current best estimates (which, to be fair, are rather shaky) support the belief that a large fraction of transmissions are made by individuals early in their own infections - over 40% within the first year, according to one study, and a large portion of that may very well be concentrated within the first portion of that year. Testing sufficiently often can address most of this (although if you're using antibody tests, the window period is an issue), but sufficiently often may prove to be far more than the once per year currently being recommended.

2. Achieving complete viral suppression takes time, even after treatment is initiated (the median is a bit under 1 years for individuals with optimal retention in care, according to a study published within the last couple years). Partial reductions in viral load prior to that point may reduce transmissibility, but they won't eliminate it.

None of this means that treatment as prevention isn't a worthy goal, or even that once a year UT&T can't achieve elimination by itself (for some plausible parameter sets it can, for others it can't). But it does mean that Dan's final sentence is not necessarily accurate.

@Confluence: If HIV+ men who want to buttfuck condom free and not disclose actually get in treatment, adhere to treatment protocols, and get their viral loads checked on a regular basis, these results suggest that that isn't a big issue, from a public health standpoint. Given the greatly elevated transmission rates during acute infection, barebacking with high-risk individuals (and if they're up for barebacking, and not in a committed relationship with you, guess what!? They're high-risk!) who recently tested negative by antibody test might very well be a worse bet. Now if people just assume their viral loads are "obviously" undetectable without getting them checked, that's another story.

The unfortunate reality is that there are a tremendous number of people who are not recieving HIV medical care, either due to insurance issues, poverty, lack of access, shame, fear of domestic violence, as well as other co-morbidities like mental health issues & substance abuse, which occur at much higher rates among HIV positive indivuals.

As Christopher has pointed out, even those that are accessing some medical care don't always achieve viral supression. Some national estimates put the number of HIV positive individuals who have achieved viral supression around 19%.

So is this a worthy goal? Hell yes... Are we anywhere close? Hell no.

With an undetectable viral load it is Extremely unlikely that he can pass on the virus. Doctors have been saying for a couple of years that an undetectable viral load provides as good protection from HIV as a condom, if not better. The Partner study has proved that.

For those who didn't read the details, The couples in the Partner study barebacked an average of one or more times a week. In the first two years of the study, NONE of the HIV Negative partners contracted HIV from their Poz partner. With an estimated 16,400 gay guy bareback f**** (and 28,000 straight ones), ZERO Negative partners caught HIV from their Positive spouse. There was a cumulative total of 894 Years of couple following-up, with not a single transmission. So who's spreading HIV. It's you "Surostatus Unknown" guys who don't get tested often enough. Literally almost all the new cases are coming from guys who don't know they are Poz. I've seen estimates as high as 54% from the newly infected acute phase guys who contracted HIV less than six weeks ago. Add the chronic infection stage men, and it totals to over 83% of new transmissions. The results from the Partner study will push that number higher. In the US you've got that special case where it's not easy for everyone to get on meds as soon as they are diagnosed (insurance, finances, etc). The diagnosed, but not on ARVs account for about 10% of new transmissions.

The most dangerous POZ guy you'll meet, was negative at his last test. It takes the body between three weeks and three months to mount an immune response to HIV. Mr. “D & D Free” who isn’t quite as D Free as he thought, became Poz several months ago and probably has a viral load between 30,000 and 120,000.
