Ok, know this is an old redux, but I think they only circled the correct answer here (in the first response, which is the one I agree with most). If a girl takes her top off completely (as in the original letter)and allows you to see, then allows you to touch her, she's probably interested in you. She may or may not be interested in full-on sex or oral, she might just want to flirt and maybe make out. I'd go ahead with the backrub, but flirt back while allowing her to steer things, or maybe take your own shirt off, since that would be likely to up the tension and you can justify it by saying it's "only fair". If she seems uncomfortable at any point, back off, since she's putting herself in vulnerable position. If you let things happen naturally, and don't push, then you're more likely to get laid eventually, if not that night. This guy was nuts- he was actually complaining about seeing a half-naked woman! On the bright side, she didn't end up wasting her time on an immature idiot.
