Or the dildo could be mounted to his face using the forhead and chin as stabilizing surfaces for the straps, so they run vertically rather than horizontally around the head. He'd have to have a hole for his mouth or a Y-shaped opening.

Seems to me that his nose is enough penetration for her to feel penetrated (and apparently she loves it), and the paucity of nerve endings in the upper parts of the vaginal canal means that she wouldn't necessarily appreciate the dildo unless she was riding up and down on it, which would lead to less constant clitoral stimulation... In other words, I'm not sure this game would be worth the candle.
