She was already extending her hand, making eye contact with and walking towards the first lady before Trump even finished shaking the Polish President's hand. Sure, Trump looked upset, but you are clearly exaggerating when you say she "denied him".
If you slow-frame through the video clip it's clear she's looking directly at IL DOUCHE's tiny little paw as he shakes her husband's hand and releases his patented mano-a-mano death grip. She's still glancing down as she crosses in front of her husband and the little orange paw begins to come up towards her, at which point there appears to be a brief moment of eye contact between them as she extends her own hand, followed by a slight tilt of her head as she averts her gaze and steps past to shake the hand of Mrs. IL DOUCHE instead.
Admittedly, it all happens very quickly, and from this angle one can't be certain, but that slight open-mouthed gape followed by the thin-lipped pout would appear to indicate he knows he's just been royally - and quite deliberately - dissed.

I actually found out today that it is considered quite rude in Poland for a man to extend his hand for a shake to a woman unless she offers first. Trump's team should probably have warned him of this, but that cultural embarrassment is on him as a leader Perhaps the First Lady was trying to spare him the gaffe, or was not expecting it due to her cultural norms.
