I'm interested in hearing more about the "discharge papers from Harborview Medical Center." It sounds to me like someone at Harborview might have overlooked (or been forced by budgetary constraints to ignore) the need for a psych hold or an involuntary commitment.

Or, third option: He didn't meet the threshold for involuntary commitment. If this guy wasn't considered crazy enough to qualify, some ear biter wouldn't either.
